Hawaii Care Choices, formerly Hospice of Hilo. Summary of services: reasonable & necessary medical care & support services for management of terminal illness are furnished under a plan-of-care established by the Hospice and the patient's attending physician.
1) Assist patient to live a full life as possible (personal care tasks, comfort measures, safety measures,
social & emotional needs, spiritual needs).
2) Provide respite for families (temporary relief from care of patient): meal preparation, shopping, errands, provide for pleasure rides, appointments, & nursing care.
3) Assist with coordination of support network: direct patient/caregivers to community resources; assist with
procurement/rental of medical supplies/equipment; contact friends, family, neighbors to help, if needed.
4) Assist with grief & bereavement needs.
