Description:
Operates an adult day health program that provides therapeutic services and treatment including recreational therapy, nursing care, and social services. Aimed for adults at risk for long term placement. A part of Hawaii Health Systems Corporation. Hours: Monday - Friday, 7 am - 5:30 pm. Saturday 8 am - 4pm Target group: persons with disabilities. Eligibility Requirements: 18 years of age or older, determined to need ICF care by private physician, possess the strength and endurance to participate in an activity program, able to partial weight bear during transfers, willing to strive for a higher level of independence, willing to attend. Application process: in-person screening interview, appointment, written application. Documentation needed: doctor's form/letter, ICF certification, medical insurance information, SS card.
